Case Manager - Rapid Rehousing - Bronx/Brooklyn in New York

OVERVIEW

The Case Manager will carry a caseload of families, individuals, or combination of both, who have criminal legal involvement and will conduct home visits on all boroughs. The Case Manager, under the supervision of the Director of Rapid Rehousing, will deliver services to achieve the goals of individual or family stabilization, independent living, and financial security.

RESPONSIBILITIES

• Conduct initial intakes and assessments and annual assessments as required.
• Monitor substance use services program treatment agreement/contract with residents.
• Provide individual services to a caseload of approximately 25i ndividuals.
• Develop initial Individual Service Plan (ISP) including relapse prevention plans with all residents on caseload.
• Conduct quarterly ISP reviews and annual ISP update with residents as required.
