Full Job Description

We are seeking a dedicated and experienced Case Manager to join our Medical Respite residential program. This role focuses on delivering recovery-oriented services to individuals transitioning from homelessness and managing complex health and social needs. The Case Manager will provide assessment, engagement, treatment planning, resource coordination, and housing stabilization support to program participants while upholding our core values of Respect, Integrity, and Compassion.

Key Responsibilities:

  • Collaborate with homeless networks, Continuum of Care organizations, healthcare providers, and state and local agencies to build robust support systems for participants.
  • Connect participants to vital services including mental health and substance use treatment, primary healthcare, income benefits, housing assistance, legal aid, education, peer support, and employment resources.
  • Conduct comprehensive assessments and develop individualized treatment plans that prioritize recovery and housing stabilization.
  • Facilitate enrollment processes, treatment planning, crisis interventions, and discharge planning in partnership with program supervisors and clinical staff.
  • Will conduct initial needs assessment within 24 hours of admission, to ensure needed services are identified and put in place.  
  • Complete Comprehensive baseline assessment to determine factors that will influence care, treatment, safety, in lieu and needed services
  • Complete Individualized Care Plan, based on needs assessment and the Individualize Care Plan, must be updated as necessary after each comprehensive needs assessment review (completed every 60 days)     
  • Ensure wellness notes are completed every 24hrs; via documentation
  • Coordinate on going services with outside providers for continuum of care
  • Coordinating transportation services for medical needs 
  • Liaison between Medical Respite and Care providers, facilitating effective service delivery
  • Connecting and engaging individuals with community health workers, and peer support services, in accordance with their needs and preferences. 
  • Participate in and coordinate, service needs, documentation flow and maintenance of electronic participant electronic record in Ehana
  • Complete and maintain discharge reports
  • Support participants in developing skills and strategies for securing and maintaining permanent housing.
  • Advocate for participants' needs and coordinate care to ensure seamless access to services.
  • Maintain accurate documentation and comply with all program policies and regulatory requirements.

Requirements

  • Education: Bachelor’s degree in human services, social work, or a related field; master’s degree preferred.
  • Experience: Minimum of six months of relevant experience providing person-centered, trauma-informed services to individuals experiencing homelessness, addiction, and/or mental health challenges. Candidates with lived experience are strongly encouraged to apply.
  • Skills:
    • Strong interpersonal and communication skills, including oral, written, and computer proficiency.
    • Ability to work independently and collaboratively as part of a team.
    • Effective time management, critical thinking, and organizational skills.
    • Capacity to adapt to the dynamic needs of program participants.
  • Certifications: Must be able to pass and maintain certifications in Medication Administration Program (MAP), CPR, and First Aid.
  • Other Requirements:
    • Valid driver’s license, reliable transportation, and sufficient automobile insurance.
    • Must be at least 21 years old and pass a CORI/background check.
