Youth Case Manager for V.I.B.E. – Vision & Independence, Building Empowerment

Location: Sussex County, Delaware (Office locations in Georgetown, DE & Seaford, DE)

Employment Type: Temporary Full-Time

Duration: March 15, 2025 – September 30, 2025

About V.I.B.E.: Join us at V.I.B.E. – Vision & Independence, Building Empowerment, a groundbreaking program dedicated to helping transition-aged youth (16-24) combat opioid use disorder (OUD) and stimulant use disorder (STUD). Our strengths-based case management approach supports holistic care and life navigation, addressing key areas such as mental health, housing, education, and employment to foster recovery and enhance long-term well-being.

Position Overview: As a Youth Case Manager, you will be instrumental in changing the lives of young individuals by providing personalized, intensive case management. This role requires a dynamic individual who can create and implement care plans that promote independence and resilience in facing life's challenges.

Key Responsibilities:

• Assess client needs and establish comprehensive care plans.
• Coordinate access to medical, educational, and employment services.
• Monitor client progress and adapt plans to ensure optimal outcomes.
• Collaborate with community organizations to enhance support networks.
• Document all case interactions and outcomes in accordance with program guidelines.

Requirements

• High School Diploma or equivalent is required.
• Bachelor’s degree in Social Work, Psychology, or related field preferred.
• Proven experience in case management, especially with youths facing mental health and substance use disorders.
• Excellent communication and relationship-building skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid Driver's License and reliable transportation required for travel throughout Sussex County, Delaware.
