The Iowa Primary Care Association is actively recruiting a Mental Health Therapist to provide care at a vibrant community health center (FQHC) in Council Bluffs, Iowa!

About the Health Center:
Join the dynamic and multidisciplinary behavioral health team at an outpatient community health center, where they provide an integrated care model that includes dental, medical, behavioral health, pharmacy, all under one roof!
Role Details:
- Clinic Hours: Mon/Thurs: 8AM - 7PM; Friday: 78AM - 5 PM; no weekends (schedule within these hours will be discussed)
- Full time position: 40 hour position
- Weekend hours may be available for additional compensation.
- Sees on average 8-16 patients per day
- Appointment types: 60 minutes for new patients, 30 minutes for follow ups
- Provider team includes: 4 therapists, 3 psychiatric nurse practitioners, 2 behavioral health patient navigators, 1 community health worker.
- Bilingual in Spanish preferred, but not required.
- EHR: Epic
Duties:
- Identifies barriers and poor social determinants of health that may influence treatment.
- Creates formulations after each initial assessment and identify corresponding diagnosis according to Diagnostic and Statistical Manual of Mental Disorder-5.
- Develops with patient a comprehensive treatment plan, utilizing evidence-based interventions that work to reduce impairment and improve functioning.
- Provide evidence based counseling and behavioral health therapy to patients who seek treatment. Psychotherapy services may include but not limited to: Addiction Counseling, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Family Systems Therapy, Grief Counseling, Group Therapy, Marriage and Family Therapy, Mindfulness-Based Cognitive Therapy, Motivational Interviewing, and Trauma Focused Cognitive Behavioral Therapy.
- Provide both short term and long term psychotherapy services in settings that include but not limited to: warm handoffs, crisis intervention, and discharge planning.
- Assumes accountability and maintains documentation for all consultation notes, significant incident occurrences, phone conversations, etc.
- Work with other departments (Medical, Dental, Pharmacy, etc.) to provide integrated behavioral health services.
Requirements
- Licensure or ability to obtain licensure in the state of Iowa as an LMHC or LISW.
Additional licensure as a LADC is preferred. - One year of full-time work-related experience in agency and/or medical clinic service delivery (counseling, case management, outreach, prevention/education, etc.)
- A minimum of two years of experience providing behavioral health therapy to individuals, families, and groups preferred.
